Life Choice provides Support Coordination services across the Fraser Coast and surrounding areas.

What does (or should) a Support Coordinator do to assist participants?

• • Build a participant’s ability to connect with informal, community and NDIS funded support providers, enabling them to get the most out of their plan and achieve their goals.
  • Help a Participant build the skills they need to understand, implement, and use their plan.
  • Help a participant to understand and manage their NDIS plan budget to ensure funds are used appropriately according to NDIS requirements and to ensure participants do not run out of NDIS funds prior to the end of their plan
  • Help participants, their families and guardians to understand what NDIS can fund and support, and what is the responsibility of other programs
  • Help participants, their families and guardian to better understand the roles, responsibilities & limitations of different support and service providers with NDIS
  • Monitoring all participant support providers to ensure they are working in a way which is complaint with NDIS requirements and appropriately using participant NDIS funds
  • Provide independent information to NDIS participants around local NDIS support and service providers with current good service reputations
  • Consult with participant NDIS provider services to ensure appropriate shift documentation and reports are being collected and written so that required evidence is being collected for NDIS
  • Consult with all participant NDIS provider services to ensure appointments and activities are appropriately coordinator and completed
  • ….and importantly to provide positive support and assistance to all participant provider services to help ensure all supports are working cooperatively together to provide the best possible support for participants

Life Choice’s highly skilled team of Support Coordinators have worked in Support Coordination since the roll out of the NDIS on the Fraser Coast. Our teams’ experience includes both lived experience and previous professional experience.
Our team has up-to- date knowledge of the NDIS policy and guidelines, and they engage in continuous professional development and training opportunities.
